Same boat! But even if you have the card, you have to get a chip reader to use it to apply anyway which seems like a pain. They should be sending out a paper form you can send in. Check your ward website! That's where I found out about it.

Like the person above said, you can get the paper form. But it's a good idea to get the card for the future since you can use it to pay taxes.
