Responsibilities / Info!

Schedule: 32 Hours / Week Guaranteed Minimum
Provides direct care to clients as needed to protect the safety of the client and maintain an acceptable level of client satisfaction.
Performs Supervisory visits, Home Safety Checks, and Plan of Care Update visits as required for all LTAC clients and provides assistance as needed to Client Care Coordinators.
Documents pertinent information in clients' clinical record, including chronological care management notes, in a clear and concise manner to reflect the care provided, the client's response to care, changes in condition, progress towards stated goals, and factors that might impact quality of services and/or client satisfaction.
Monthly supervisory visits to VA Vent and Trach Clients to make sure client concerns are addressed and nurses are compliant with client's needs.
Reviews skilled nursing care plans and daily nursing notes to make sure they properly assess needs of clients, meet standards of the agency, and are in hands available for timely processing for billing, and available for filing in charts.
Provides patient education to clients, family members, and coaching to Right at Home staff (skilled and unskilled), to promote safe and appropriate care whether or not Right at Home staff is in attendance.
Assists in annual skills assessment, performance evaluations, and coaching and mentoring of clinical and caregiver staff as required.
Complies and implements individual in-home care Plans of Care (POCs) for pending clients as assigned based on physician orders and in-home assessments.
Applies nursing assessment skills, judgement and effective written and verbal communication skills to open new Home Health and Home Care cases and maintain a existing case panel of home care and skilled nursing clients.
Assists clinical operations team to ensure agency is meeting standards outlined in its policy and procedure manual which includes Joint Commission standards, and Washington State Home Health regulations (WAC 246-335-400 and 500).
Conduct 6 month in-field reviews of all agency nurses in collaboration with Director of Nursing and assist with reviews of all home care nurses.
Reviews nursing notes for changes in condition and works with Right at Home Care Team to obtains orders for changes to the Plan of Treatment and makes sure all observations and interventions are documented in the clinical record.
Provides supervision, support and instruction to Clinical and Home Care staff.
Complete a minimum of (10) ten hours of training annually on topics related to supervisory duties and the delivery of home care and home health services.
