Job Description MHS is a non-profit organization whose mission is to improve people’s lives and instill hope by providing cost-effective community- based mental health, drug and alcohol recovery, and housing services. PROGRAM SUMMARY Action East (Assertive Community Treatment) is a dual track treatment program for individuals with severe mental illness and/or a chronic substance use disorder. The program provides therapeutic interventions, medication management and monitoring, comprehensive outpatient and community services, case management, employment support, peer support and supportive housing services. JOB SUMMARY Case Managers offer their community outreach abilities and extensive knowledge of community resources and support as a primary means of assisting each client with achieving long-term recovery, housing, and employment. RESPONSIBILITIES Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the job. Prepare and maintains assessments, comprehensive service plans, progress notes and other records and documentation. Gather information for intake assessment and housing placement. Conducts intake interviews and completes individual assessments of client’s status and level of required comprehensive services. Conduct bio-psychosocial and/or housing screenings and SPDAT Assessments and utilizes the Coordinated Entry System (CES) database as directed by the Program Manager. Make appropriate referrals to community providers coordinating and arranging access to care with ancillary services. Facilitate placement including arranging pre-placement visits and transportation, or provide transportation as needed. Conduct chart and treatment plan reviews to ensure compliance with agency, federal, state and local government standards, and monitor and evaluate client progress and changing needs throughout treatment. Develop and maintain a caseload as assigned by the Lead Substance Abuse (AOD) Counselor and Program Supervisor and maintains a daily log of client contacts. Promote and conduct outreach and engagement, Housing First ideologies, and acts as a liaison between landlord and tenant. Transports clients in MHS or personal vehicle as required. Provide community outreach and early intervention activities by supplying information about the program to community agencies connected to the SUD services system in order to educate them about program availability. Provide support to clients and families and encourage family members and client significant others to participate in client stabilization and counseling services, referring those needing specific services and support. Attend Homeless Management Information System (HMIS – Service Point) training and participates in data gathering activities and running reports as directed by the Program Manager Maintain contact with assigned treatment providers to continuously appraise client progress and provide crisis intervention. Promote effective relationships with staff by demonstrating a coordinated team approach to treatment and prepare critical care issues for discussion during supervision. Establish weekly case presentations for admission review, including all admissions in the caseload. Intervene for non-open cases in a crisis situation by referring to mental health or co-occurring services. Inform at-risk populations about the need for and availability of Medi-Cal and non-Medi-Cal programs, and other duties as assigned. Aid in general management of center including running errands as assigned. Complete all assigned duties within the time frame and according to the standards as established by the Program Manager. Comply with all MHS and program rules, policies and procedures including HIPAA standards. QUALIFICATIONS Possess or be in the process of obtaining certification for substance abuse treatment, as CCAPP, CAADE, or similar professional certification Minimum of two years of experience providing substance abuse treatment and care to adults In-depth knowledge of substance abuse treatment methodologies and diagnostic techniques. Familiarity with San Diego substance abuse services system Valid California driver’s license and current auto insurance per contract requirements Proficient with Microsoft Office and EHR software Ability to problem solve and react appropriately to emergency situations Ability to work independently and effectively collaborate with a teaM OTHER DUTIES Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job.
