Responsibilities

This telehealth clinical liaison will work as a part a multidisciplinary health care team, focusing on the needs of the patient. This person would be responsible for facilitating the Telehealth sessions between patients and their clinical provider. Acts as the liaison between clinical staff and the patients before and during their medical appointments. Is responsible for the and notification of appointments scheduled to our clients and for educating clients on how to use the equipment and platforms to connect to their virtual consults.

Responsibilities and essential duties:

1. Provides technical assistance to patients on how to navigate our telemedicine and electronic record platform for their virtual medical visits and offers additional support to the medical team to conduct chart reviews for patients with upcoming medical appointments. This includes patient registration, insurance eligibility, appointment verification, rescheduling, and the intake of other general telephone inquiries.

2. Assist behavioral health professionals during telemedicine appointments to navigate clients and staff to appropriate virtual rooms.

3. Call patients to confirm and get consent for telemedicine visits prior to their appointments.

4. Proactively supports providers and staff for upcoming virtual health initiatives, including support for possible technical issues.

5. Educates patients and their relatives on how to access and navigate patient portal and telemedicine platform. Make sure patient can connect to virtual appointment.

6. Person should be familiar with organization telemedicine and telehealth policies and keeps informed of changes.

7. Coordinate with the medical staff and therapists’ schedules for telemedicine appointments.

8. Maintain patient charts and all necessary data entry in a timely manner.

10. Maintain open lines of communication with patients, providers, and other areas/departments.

11. Documents in electronic record relevant clinical information.

12. Act as first contact for patient in non-emergency situations such as med refills or office appointments.

13. Person demonstrates knowledge and ability work with diverse populations in the community.

14. Complete reports monthly as to the number of services provided to meet or exceed quality measures or requirements.

15. Receives and verifies the client’s referral for coordination of appointments and requests additional information, if necessary.

16. Completes coordination of care for the clients and dependents, within the timeframes.

17. Contact clients or its relatives to notify the appointments coordinated within the timeframes.

18. Participate in meetings and activities, as required.

19. Shows appropriate communication skills, effective listening, and management skills to deal with difficult clients.

20. Keeps the specialization license and professional association membership (colegiación) effective

21. Complies with the continuing education hours that corresponds to the specialization.

22. Participates in the mandatory trainings, such as: Annual Compliance Plan and CMS guidelines and regulations.

23. Informs the supervisor or human resources of any sanction or revocation of your license that affects the practice of your profession.

24. Available to travel throughout Puerto Rico.

25. Other tasks assigned by the supervisor.

Qualifications
• Education: Master’s in social work, Psychology, Psychological Counseling, Mental Health Counseling, or Nursing
• Licenses: Valid and current active license for Social Work, Nursing, Psychological Counseling, Psychology, or Mental Health Counseling
• Professional associating membership (colegiación): Valid membership for Social Work and Nursing
• Relevant Work Experience: Should have at least two or more years of prior clinical experience.
• Knowledge in Microsoft Office
• Excellent communication skills
