Job Description
Job Duties
• Consultation:
• Receives referrals from ED team for consultation thru EPIC
Assessment and Evaluation
• Conducts substance use assessments to determine educational needs and to recommend to Care Coordination possible discharge resources to clinically appropriate patients, including possible admission to the Bridge Clinic.
Interventions
• Creates a positive therapeutic rapport with patients while providing individual mental health, crisis and substance abuse interventions.
• Uses motivational interviewing to assist the patient in realization of problem and need for change.
• Determines if the intervention of a peer support specialist is needed and works closely with peer support staff to ensure patients’ needs are met.
Resource Identification:
• Identifies hospital and community based services which will meet patient/family needs.
• Initiates and completes referrals to community resources on behalf of patient and advocates in obtaining services.
Patient/Family Education
• Provides education to patients and families to empower and motivate patient and enhance problem solving and coping skills.
Other Social Service Needs
Assesses and reports child and adult abuse/neglect;
Offers grief counseling to families related to death/dying or to accepting chronic and terminal illnesses;
Assists with discharge planning activities for those patients being discharged from the hospital.
Documentation:
• Documents assessment and all interventions in the electronic medical record.
Personal Growth and Development:
• Maintains skills and knowledge as appropriate to the medical social work role. Participates in continuing education to fulfill licensure requirements.
Performance Improvement:
Assists in the process to evaluate patient outcomes, financial and clinical results that can be tracked to measure success of the Social Service Department.
Qualifications
MINIMUM
DESIRABLE
Education, Credentials, Licenses:
MSW from accredited school by the Council on Social Work Education
Licensed as Master’s prepared social worker in state where work is being performed.

LICDC - License Independent Chemical Dependency Counselor
Specialized Knowledge:
Knowledge of emotional and mental disorders and mental disorders as related to DSM V criteria and a knowledge base of psychodynamics, human relations, crisis intervention, psychopathology, and group dynamics.
Knowledge of Opioid epidemic and resources available in our community.
Excellent communication skills and the ability to coordinate care well with an interdisciplinary team.
